Children's community nursing service

Overview

The children’s community nursing service provides expert nursing care to enable children and young people to stay at home with their family to prevent admission to hospital or early discharge.

The service also aims to support families in the care of children and young people with long-term conditions or complex nursing needs and end of life care when necessary, as close to home as possible. The service provides training to carers, schools and other healthcare professionals in partner organisations in understanding and managing specific healthcare needs and empowers families to confidently care for their child.

The service is comprised of six teams:

Depending on your child’s care needs, they may be treated by a combination of teams to provide a holistic service.

We regularly have student nurses on placement with the team. If you do not wish a student to be present at visits, please let your community nurse know.

Who is this service for?

Children and young people aged 0-16 (or 0-19 for those attending named local complex needs schools) who are registered with a GP in Lambeth or Southwark

To order more supplies, please contact the team on 020 3049 5858.

Our supplies line is open between 9am-12pm Monday to Friday. Any messages left after 12 noon will not be dealt with until the following morning. Please note, the team need 7 to 10 days’ notice to order supplies. If you need supplies as an emergency, please contact the team and collection can be arranged from Sunshine House. You must make arrangements with the team first.
