Children and Young People can be referred to the service by other medical and health care professionals.
Referral criteria for the community nursing team
We support families to care for children with a nursing need in their own homes to reduce the need for admissions to hospital. We aim to respond to referrals within 24 hours and see children between the hours of 8am-6pm
We see conditions such as, but are not limited to:
- intravenous, intramuscular and subcutaneous medication administration
- enteral feeding support and care with naso-gastric tubes
- enteral feeding support and care with gastrostomy and jejunostomy tubes
- central line bloods and care of central lines
- oncology
- tracheostomies and home ventilation
- sleep studies, oxygen therapy, saturation monitoring
- wound care
- blood pressure monitoring
- palliative care and symptom management
