REACH service referrals

 

Who can refer

We accept referrals from medical practitioners, such as:

  • consultants from Evelina London or other hospitals
  • child development centres (for example, community paediatricians, neonatologists and neurologists)
  • general practitioners

Referral criteria

Children who live in the southern boroughs of London, Kent, Surrey or Sussex are eligible for consultation and advice appointments, and where clinically indicated, our intensive rehabilitation programmes. For children outside these areas, we can offer one-off consultation and advice appointments. We encourage local therapists to attend these appointments.

Children do not need to be under the care of a medical consultant at Evelina London Children’s Hospital to be referred to us as we accept external referrals directly.

How to make a referral

Referrals (that include relevant medical history) can be emailed to gst-tr.EvelinaREACHReferrals@nhs.net

  • our rehabilitation model
  • the results of our programme
  • the role of community based therapists
  • the young people and their parents or carers in our programmes
