Referrals should be made by local paediatricians to the pan-London post-COVID hub via email london.cyppostcovid@nhs.net
Please send a referral letter along with this completed form (Word 130Kb).
Please signpost your patients/families to the “NHS Your Covid Recovery” website. It has information and guidance about some of the different symptoms of long COVID tailored for children and young people, and it explains where they can find further help and support.
