The compass service

What is the compass service?

Being in hospital can be a worrying time for children and families. For those children and young people with a range of different health needs, it can be particularly confusing. They may be under the care of multiple health professionals, each responsible for a particular specialist area, but sometimes that care isn’t as well coordinated as it could be. In response to feedback from families, we have created a dedicated compass team to help join up care.

The compass team aim to work with the wider team to provide better communication, coordination and continuity of care during children’s stay in hospital. The team is made up of a general paediatric consultant and clinical nurse specialist. We work closely with our Well Child discharge coordinator.

Depending on the situation, we may carry out a one off review with advice to the lead medical teams and signposting to other resources. At other times, we may remain involved throughout the entirety of the hospital stay and aid with transition from hospital to home.
